To get insured, you should fill out

1. one of the following insurance forms (according to your status)

• Insurance form for students with family health insurance
• Insurance form for students without family health insurance

     2. the following Excel form

• summer_practice_insurance_info.

You should send the insurance and Excel forms together to ie-staj@metu.edu.tr 10 days (3 days at the latest) before your summer practice starts. Please note that you send the forms no earlier than 10 days and no later than 3 days before your summer practice.

You are not allowed to make changes to the information you have sent, so before sending the forms, be sure that you filled out all the fields correctly. Note that it is very difficult to change the submitted information. If you need help with your data entries, you can send an e-mail to ie-staj@metu.edu.tr.
